Exercise Physiology Referrals

Our Allied Health Hub offer a range of services for Exercise Physiology (EP) including individual and group sessions, home visits, gym, hydrotherapy, stretch sessions, advice and guidance relative to specific neurological conditions. We provide a neurogym specifically designed for neurological conditions and neuroplasticity.

Kate Dean, our Director and Principal Clinical Exercise Physiologist is a Professional Allied Health Practitioner with close to 40 years experience. Kate’s special Interests include Complex Rehabilitation, Complex Medical Comorbidities, Rare and Complex Genetic Conditions, Cardiopulmonary Disease, Chronic obstructive pulmonary disease (COPD), Cardiovascular Disease (CVD), Fibromyalgia, Neurological Conditions and Neuroplasticity, Acquired Brain Injury and Stroke.

Service Overview

Exercise Physiology

Our Accredited Clinical Exercise Physiologists provide holistic individualised assessments, followed by the design, implementation and evaluation of targeted short and long term exercise interventions that are designed specifically to improve your overall health and wellbeing. Your Exercise Physiologist will work with you to improve your overall strength and mobility, along with chronic disease management and prevention, musculoskeletal injury rehabilitation, education, support and guidance to reach your goals.

Hydrotherapy

Our Allied Health Hub offers regular group and individual Hydrotherapy sessions to improve mobility, health and wellbeing through the implementation of individualised low impact exercise. With a positive retention rate, our participants consistently enjoy the energetic and positive atmosphere, social support and their exercise programs.

Neuro Gym

Our Neuro Gym has been designed specifically based on the clinical principals of neural brain training, this body to brain training relies on neuro plasticity, the brains response to specific targeted exercise, clinically developed by our Exercise Physiologist for effective neural impact.

Check with your Allied Health team to see if extra sessions can be scheduled with our Allied Health Assistant. 

Occupational Therapy*

Our Occupational Therapy service provide assessment of your current needs, functional ability and goals. Your Occupational Therapist will provide solutions to improve your capacity to engage in activities of daily living such as adaptive or modified tools, mobility equipment, including powered wheelchair and scooter prescription along with assistive technology to improve your safety, mobility, home and community access.

Counselling*

Counselling provides communication based support and guidance to overcome challenges and make positive changes for your mental health and physical wellbeing.

Our Counsellor is equipped to provide Social Work Support, Behavioural Modification and Life Coaching.

Allied Health Assistance

Our Allied Health Support Team contributes to your overall care, goals, access. They support your Multidisciplinary team of Accredited Allied Health Professionals in the provision of care and interventions. They are trained by your Allied Health Professional to support specific interventions or education and provide cost effective training, sessions, additional program sessions for ongoing implementation.

Funding Information

We are passionate about access to health services for all, with funding options available and no gap fee we believe all people should have consistent and affordable access to health care and interventions. 

Medicare, NDIS, WorkCover, DVA, Commonwealth, Insurance and Private Patient fees available.

  • Your GP can provide you with an Enhanced Primary Care Plan (EPC), you may be entitled to up to 5 Medicare Funded Exercise Physiology sessions annually. The standard session length is 20 minutes.

  • Your GP can provide you with an Chronic Disease Management Plan (CDM), you may be entitled to Individual and Group Exercise Physiology sessions or programs funded by Medicare. The standard session length is 20 minutes.

  • If you are an NDIS Participant you may have Core Funding for Allied Health and Supports, or you may be able to access Capacity Building Supports through a team of Allied Health Professionals like Our Allied Health Hub, assessments, reports, and interventions to build strength, improve mobility, increase independence, skills and access to your community and to assistive technologies.

    We draft comprehensive reports for NDIS Participants.

  • TPI Gold Card holders, for which a new referral has been issued, daily consultations can be claimed in the referred 12-month period and are provided to the client individually with an expected appointment duration of at least 20 minutes. Only one allied health consultation may be claimed per day and the treatment must relate to DVA accepted condition/s.

    Treatment for White Card holders must be related to an accepted disability. Eligibility must be established before starting treatment and there are 12 consultations assigned to each referral, a new referral is required at the end of each cycle to continue care.

  • You may be eligible for a Chronic Disease Management Plan (CDM) referral from your GP if you require care from a Multidisciplinary team. For example Exercise Physiologist and Occupational Therapist or Speech Pathologist.

    A chronic medical condition is one that has been or is likely to be present for at least six months, e.g., asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke. A patient is considered to have complex care needs if they require ongoing care from a multidisciplinary team consisting of their GP or medical practitioner and at least two other health or care providers

  • The Commonwealth home support program (CHSP) and Home Care Packages can be an option if you need a coordinated approach to the delivery of your help at home, these are support programs for older people to help them stay at home. It assists with home care support such as home modifications, transport and help with home duties such as cleaning or mowing. It assists people to maintain their independence. Adding to this regular allied health appointments such as Exercise Physiology help you to improve your ability to be stronger, improve your mobility and manage your medical conditions well. It assists you to stay independent as long as possible and can help you manage your pain, rebuild and maintain physical function, increase your activities of daily living and keep you living your life, your way.

    At Our Allied Health Hub we offer education, advice and a space to come to perform regular exercise that meets your needs and guides you towards improved outcomes of health and well-being. We are an evidence-based practice, and we design your program according to evidence such as pathology or radiology reports and information from your GP on your medical conditions, illness or injury.

  • Allied health providers are an essential part of an employees WorkCover compensation treatment throughout the rehabilitation process by providing treatment, rehabilitation and facilitating the return to work goal as quickly and safely as possible.

    Insurances such as after a motor vehicle accident injury may cover you for Exercise Physiology. Your GP can refer you and after an initial appointment and assessment we may negotiate with your insurance company with the goal for you to engage in therapy with Our Allied Health Hub.

  • If you have the correct healthcare cover, you may be eligible for a rebate from your health care agency. Each private health insurer has their own requirements and guidelines, please call your Private Health agency and ask if you are covered for services by Allied Health Professionals.
